Oral Care with Dementia: Gentle Techniques for Caregivers

May 22, 2025

Oral hygiene is an important but often overlooked part of caring for a loved one with dementia. Brushing teeth, flossing, and maintaining regular dental visits can become confusing, frustrating, or even frightening for someone with memory loss — yet these tasks are critical to maintaining overall health.

At Story Cottage, our caregivers provide dignified, personalized support for oral care as part of each resident’s daily routine. If you're caring for a loved one at home, these gentle techniques can help make oral hygiene more manageable and comfortable — for both of you.

Why Oral Health Matters in Dementia Care

According to the Alzheimer’s Association Greater Indiana Chapter, poor oral health can lead to infections, discomfort, difficulty eating, and increased behavioral challenges in people with dementia. Routine care may become difficult due to memory loss, loss of fine motor skills, or a fear of unfamiliar sensations.

Being proactive with gentle, consistent care can prevent long-term problems and contribute to a better quality of life.

Gentle Techniques for Caregivers

1. Establish a Predictable Routine

Perform oral care at the same time each day — ideally morning and evening — in a calm, familiar environment. Use visual or verbal cues like, “It’s time to brush our teeth,” or model brushing your own teeth side-by-side.

2. Use Simple, Clear Instructions

Offer step-by-step guidance. For example:

  • “Let’s hold the toothbrush.”
  • “We’re going to brush the top teeth first.”
Use short phrases, a calm voice, and lots of encouragement.

3. Choose the Right Tools

  • A soft-bristled toothbrush or an electric toothbrush with a gentle setting
  • Toothpaste with mild flavor (strong mint can be unpleasant or too intense)
  • Fluoride mouth rinse or spray (if tolerated)
  • Toothette sponges or oral swabs for those unable to tolerate a toothbrush

4. Use Hand-Over-Hand Assistance

If your loved one struggles to brush independently, gently place your hand over theirs to guide the motion. This technique promotes independence while providing support.

5. Reduce Discomfort and Fear

If your loved one resists brushing, try using a warm washcloth to gently wipe the teeth and gums, especially before bed. Playing soothing music or explaining each step calmly can help ease anxiety.

6. Watch for Signs of Oral Pain

Look for signs of discomfort such as:

  • Refusing to eat or drink
  • Touching the mouth or face frequently
  • Bad breath or visible plaque buildup
  • Sudden behavioral changes

These may signal tooth decay, gum disease, or an ill-fitting denture — all of which should be evaluated by a dentist familiar with dementia care.

Regular Dental Visits Are Still Important

As dementia progresses, regular check-ups with a dentist who understands cognitive impairment are essential. You might consider:

  • Visiting a geriatric or special needs dental practice
  • Requesting shorter appointments during your loved one’s calmest time of day
  • Bringing familiar items like a favorite blanket or music

Your local Area Agency on Aging or the Alzheimer’s Association Greater Indiana Chapter can recommend dementia-friendly dental providers in your area.
